To receive your rebate, simply follow these steps: 1) The qualified rebate is valid only for purchases of the Qualifying Models made between July 1 and September 30 2010. 2) Read the complete terms and conditions for this rebate below. 3) Complete this rebate request form. 4) Enclose a copy of the receipt showing the purchase of an eligible LG headset from the Qualifying Models listed on this form. 5) Cut off and include the original UPC Code from the Qualifying Models LG product box. 6) Make a copy of all materials submitted for your records as items will not be returned.
