How to Claim Your Blockbuster Rental Card and Receive Your 2-Year Limited Warranty: 1. To receive your $400 BLOCKBUSTER rental card and 2-year limited warranty, purchase a new Panasonic PT-AE2000U projector from an Authorized Panasonic Dealer in the United States (or PT-AE2000U-EC from Panasonic Direct), between October 1, 2008 October 31, 2008. This cannot be combined with any other offer. 2. Return this completed form, including the serial number label, along with a copy of your invoice with the purchase price circled. IMPORTANT: You must cut out and include the original label from the carton, which includes the UPC code, model number and serial number, or your claim cannot be processed. 3. Offer valid for purchase made from October 1, 2008 to October 31, 2008. All claims must be postmarked within 30 days of purchase. 4. This offer is only available to the end user and original purchaser.
