Terms of Offer: 1) Time period: Nikon Mail-in rebate is valid only for retail sales of new Nikon Coolpix 2100, 3100, 4300 and 5700 Digital Cameras purchased from Nikon Inc. by Nikon authorized dealers or distributors and sold to consumers by Nikon authorized dealers or resellers from February 1, 2004 through March 31, 2004. All applications must be postmarked no later than April 30, 2004. Applications postmarked after that date will be rejected. 2) Subject to fulfilling all other terms of this offer, Nikon Inc. will send a rebate check directly to the retail customer in the amount indicated on reverse. 3) Purchase confirmation: This application must be completed, signed and mailed by the applicant. It must be returned to Nikon Coolpix Rebate, P.O. Box 6001, Melville, NY 11775-6001 with: A) a copy of your bill-of-sale (do not send original) showing purchasers name, address, date of purchase, item(s) purchased, Nikon products serial number, and stating that the item has been delivered to the purchaser, and your bill-of-sale must also indicate the name of the dealer from whom you purchased the product(s); B) The original UPC code label from the side panel of the Nikon product box; and C) The original serial number label with the bar code from the side panel of the Nikon product box. 4) Who qualifies: Offer is open to any bona fide retail customer who meets the terms of the offer. Nikon camera dealers, distributors and resellers, their employees and families, as well as the employees, agencies, representatives of Nikon Inc. and its affiliates, and their families, are specifically excluded from this offer. 5) This rebate is offered exclusively: A) By Nikon Inc. only for new Nikon purchases made and delivered in the 50 United States, Puerto Rico and the Virgin Islands. Purchases made in or delivered to other countries are ineligible. B) Nikon Products sold by other Nikon subsidiaries throughout the world are not eligible for this rebate. C) For new eligible Nikon Coolpix 2100, 3100, 4300 and 5700 Digital Cameras sold to an end user retail customer. To be eligible, product must have a E2100(U), E3100(U), E4300(U) or E5700(U), legend included in the UPC code label on the Nikon product box. Products with a different legend, or without a legend, in the UPC code label on the Nikon product box are not eligible for rebate. 6) Subject to verification, each rebate check will be sent directly to the applicant within approximately six weeks after Nikon Inc. has received and verified this application, copy of bill-of-sale, and the UPC code label and serial number label from the side panel of the Nikon product box. 7) Nikon Mail-in rebate cannot be combined with any other Nikon offer. Available only at participating Nikon authorized dealers or resellers. Void where prohibited, taxed or restricted by law. 8) This offer may be withdrawn without notice. Misuse of this application constitutes fraud. Your Nikon dealer or distributor is not authorized to change the terms of the offer.
