The label you submit should have the following pieces of information:

1- Fujitsu Part Number

2- Fujitsu Model Number
Depending on where you purchased your Fujitsu Lifebook notebook it may come in two boxes. The label you want to use is on the actual notebook box (inside box if your unit comes in two boxes) and matches the sample shown. Custom configurations will not have a UPC code. If your unit was shipped in two boxes send in the label from the inside box that shows the Fujitsu part number and model number. Some notebooks will contain a shipping box that does not have all of the information needed to process your rebate. Usage of the shipping label will result in your rebate request being denied.
