Children's records. (1) At the time of the child's initial attendance, a child information card, using a form provided by the department or a comparable substitute, filled out by the parent, including written permission, signed by the parent, to seek emergency medical care shall be obtained and kept on file and accessible in the center.
Eleven of thirteen child information cards reviewed were incomplete. R 400.5111b Health care policies and resources. A written health care plan shall be implemented that includes the following: (a) Health practices and policies including procedures for the following: (i) Children and staff hand washing. (ii) Handling children's bodily fluids. (iii) Cleaning and sanitizing of all equipment, toys and other surfaces. (iv) Controlling infection, including universal precautions. The center did not have a written health care plan. R 400.5113a Emergency procedures. (3) The center shall establish and implement a fire drill program for staff and children consisting of at least 1 fire drill quarterly to assure prompt evacuation of the building in case of emergency. Only three fire drills were practiced in 2009. R 400.5113a Emergency procedures. (4) The center shall establish and implement a tornado drill program consisting of at least 2 tornado drills during the months of April to October to assure prompt emergency procedures in the case of a tornado. Only one tornado drill was practiced for 2009. R 400.5603 All motor vehicles. (4) There shall be no loose or heavy objects in the passenger compartment of any motor vehicle. There was a loose crate and trash can on the center bus.

Hazard Areas. (4) Heating shall be by a central heating plant, or an approved permanently installed electrical heating system. If heating is provided by a central heating plant and located on the same floor that is used for child occupancy, it shall be installed in an enclosure providing not less than a 1-hour fire-resistive separation, including a "B" labeled fire door and frame assembly equipped with an approved self-closing device and positive latching hardware in any interior door opening. Door openings for heat plant enclosures not located on the same floor that is used for child occupancy may have 1 3/4 inch solid wood core doors having positive latching hardware and an approved selfclosing device or equivalent. Air for proper combustion shall be provided directly from the outside through a permanently opened louver or noncombustible duct. The storage of combustible materials in a heating plant room is prohibited.
There were several cardboard boxes containing pop stored in the heat plant room. R 400.5825 Hazard Areas. (9) The center shall not store flammable materials, including fuels, pressurized cans, cleaning fluids and supplies, polishes, and matches, in heat plant enclosures. These items may be stored outside of child use areas in metal cabinets or storage facilities accessible only to authorized personnel. There was a large plastic container of paint stored in the heat plant room. A corrective action plan was requested and approved on 12/02/2009. It is expected that the corrective action plan be implemented within the specified time frames as outlined in the approved plan. A follow-up evaluation may be made to verify compliance. Should the corrections not be implemented in the specified time, it may be necessary to reevaluate the status of your license. III. FEES FOR MEDIATION
When mediation is ordered by the Jackson County Family Court to address issues of child custody and related parenting issues, a mediator from the court child custody panel is appointed by the Court and the fees paid for mediation are set by the Court. Each partys fee is individually set according to the gross income information provided to the Court on the Form 15, Response to Rule 68.12- Notification of Child Custody and Related Parenting Issues. The fees are determined using the sliding fee scale below. Parties are required to participate in a minimum of two hours of mediation and to pay the mediator for two hours of mediation before the mediation begins. When mediation addressing child custody and related parenting issues continues beyond the initial two hours, the fee continues to be paid to the mediator at the hourly rate set by the Court. Each party receives information about the mediation fees in writing before the mediation begins. If a party has an attorney, information about mediation fees is sent to the party and the attorney. If a party(ies) fails to appear for any mediation session without reasonable notice, the Court may award attorney's fees and/or costs or impose any other appropriate sanction by law. (Rule 68.12)
