Terms and Conditions: 1. Pay As You Go feature phones (LG 240, Motorola V190, Motorola V3, Motorola V360, Sony Ericsson W200a, Pantech C3; and Nokia 6085 phones purchased from The Source) must be activated or upgraded on a Pay As You Go price plan between December 1, 2009 and December 31, 2009 to be eligible for rebate. 2. Account must be active for a minimum of 30 days before a rebate cheque is issued. If upgrading, rebate cheque will be processed 30 days after date of purchase. 3. All rebate forms must be postmarked no later than January 31, 2010 to be considered for redemption. 4. Limit of two rebates per household. 5. This offer is non-transferable and is void where prohibited by law. 6. Cannot be combined with any other offer. Phones purchased from Wal-Mart, Best Buy, Future Shop, Zellers or London Drugs are not eligible for this rebate offer. 7. Pay As You Go feature phones activated on Postpaid plans are not eligible for the rebate. 8. All proof of purchase receipts (Receipts) must be legible, fully detailed copies, which include description of purchase, date of purchase, location of purchase and amount of purchase. 9. All Receipts must be either preprinted or stamped with an authorized Rogers Wireless distributor name. Receipts will not be returned.
10. UPC code, Debit Card, Credit Card receipts or Credit Card and Bank Statements are not considered acceptable Receipts to qualify for this rebate offer. 11. This offer is valid on new LG 240, Sony Ericsson W200a, Motorola V190, Motorola V3, Motorola V360 or Pantech C3 phones only; and on Nokia 6085 phones purchased from The Source. Refurbished phones are excluded. 12. Promotional offer is available to Canadian residents only. 13. All redemption requests must be sent directly by mail to the promotional PO Box indicated on the reverse side, by the customer. 14. This rebate offer is available only in accordance with these terms and conditions. 15.
