To receive the rebate, simply fill out the attached coupon and mail with receipt to Numark. All claims must be postmarked no later than 1/15/06 and received on or before 1/31/06. Numark will not honor delayed submissions due to dealer's being out of stock on the AVM01, DVD01 or VM03. Proof of purchase required (photocopies accepted).
