How to Receive Your FREE Lamp: 1. 2. To receive your FREE Lamp, purchase a new Panasonic PT-LB90NTU, PT-LB90U, PT-LB78VU, PT-LB2U or PT-ST10U projector from an Authorized Panasonic Dealer in the United States, from January 1, 2011 March 31, 2011. This cannot be combined with any other offer. Return this completed form, including the serial number label, along with a copy of your invoice with the purchase price circled. IMPORTANT: You must cut out and include the original label from the carton, which includes the UPC code, model number and serial number, or your claim cannot be processed. Offer valid for purchase made from January 1, 2011 March 31, 2011. All claims must be postmarked within 30 days of purchase. This offer is only available to the end user and original purchaser. No claims from re-sellers, dealers/distributors, their employees or warehouse facilities, groups, clubs or organizations will be accepted or acknowledged. Product must be new and un-used.
