Either way, you must post your completed claim form, along with your original purchase receipt showing model number purchased, within 21 days of your purchase date. You must include your serial number, so please ensure your retailer will deliver your TV to you in time to include this number with your claim. We will return your original purchase receipt with your Blu-Ray Player but recommend you keep a copy of your receipt for your records.
