SONY VAIO GRX SETTLEMENT CLAIM FORM
If you purchased or received as a gift a Sony VAIO GRX Notebook Computer in the 500 or 600 series, you may be entitled to benefits if you paid to repair the motherboard of your computer or if you obtained an estimate to repair the computers motherboard and chose not to have a repair completed. If you are a representative, assign, heir, executor, administrator, or custodian of the intended recipient of this Claim Form, you should complete this form because you may be eligible to receive benefits on his or her behalf. If you are submitting this Claim Form in a representative capacity, please include proof of your authority to act on behalf of and to bind the person on whose behalf you are acting.
You may be eligible for settlement benefits if you paid to have your computer motherboard replaced or the memory socket(s) resoldered. You may also be eligible for settlement benefits if you obtained a written estimate of the cost to have your computer motherboard replaced or the memory socket(s) resoldered, but elected not to have the repair performed. In either case, you are only eligible for benefits if your computer experienced failure to boot, inability to access installed memory, freezing or other related symptoms. By executing this claim form, you are attesting under penalty of perjury that your computer experienced these symptoms, and that your computer did not fail due to accidental damage, misuse, or abuse. The settlement encompasses the following specific model numbers of Sony VAIO GRX Notebook Computers, including configure-to-order models within the Sony VAIO GRX 500 and 600 series: PCG-GRX500 PCG-GRX510 PCG-GRX600 PCG-GRX520 PCG-GRX550 PCG-GRX650 PCG-GRX560 PCG-GRX570 PCG-GRX670 PCG-GRX580 PCG-GRX590 PCG-GRX690
Settlement benefits will be paid by check. Settlement checks will remain valid and negotiable for ninety (90) days from the date of issuance. If you are eligible to claim benefits for multiple computers, you must complete and submit a separate Claim Form for each computer.

Mailing Instructions

You may show dated Proof of Purchase by submitting one or more of the following: (i) copy of original receipt; (ii) copy of an invoice marked paid; (iii) copy of a canceled check contemporaneously identifying the purchase; (iv) copy of credit card bill that identifies the purchase; (v) copy of shipping invoice identifying the purchase; or (vi) other contemporaneously generated evidence confirming your purchase of an eligible VAIO GRX Notebook computer. Proof of Repair or Repair Estimate In order to obtain benefits, you must submit documentation that you paid to have your computer motherboard replaced or the memory socket(s) resoldered, or that you received an estimate but declined to obtain such a repair, unless Sony has a record of your repair. Sony would normally have a record of your repair if you obtained a repair directly from Sony. You may show Proof of Repair by submitting one or more of the following, specifically identifying the motherboard replacement or memory socket resolder, dollar amount of repair, date of repair, and name/address of the third party servicer: (i) copy of original repair receipt; (ii) copy of an invoice marked paid; (iii) copy of a canceled check contemporaneously identifying the repair; or (iv) other contemporaneously generated evidence confirming a motherboard replacement or memory socket resolder of an eligible VAIO GRX Notebook computer. You may show Proof of a Repair Estimate by submitting a copy of the original written estimate received from a bona fide PC or electronics servicer that specifically states the estimated dollar value of repair and that the repair would be either a motherboard replacement or memory socket resolder of an eligible VAIO GRX Notebook computer. Declaration Must be Completed to be Eligible for Settlement Benefits I have received notice of the class action settlement in this case and I submit this Claim Form under the terms of the proposed settlement and subject to final court approval. I also submit to the jurisdiction of the California State Superior Court, County of San Diego, with regard to my claim as a Class Member and for purposes of enforcing the release of claims stated in the settlement agreement. The full and precise terms of the proposed settlement are set forth in the Settlement Agreement. I further acknowledge that I am bound by the terms of any court judgment that may be entered in this action and may not bring any separate litigation against Sony Electronics Inc. or against related entities released in the Settlement Agreement related to this action, or that could have been asserted in this action, as set forth in the Settlement Agreement.
