To qualify for this benefit you must (1) have a Sony television model KDS-R50XBR1 or KDS-R60XBR1 and (2) have purchased a 1080p DEVICE (see definition of this term below) on or before July 25, 2008. To qualify for this benefit you must (1) have a Sony television model KDS-R50XBR1 or KDS-R60XBR1 and (2) have purchased a Sony Blu-ray Disc Player (excluding gaming device) between July 25, 2008 and 30 days after the Court grants final approval. (To find out the date the Court granted final approval, go to http://esupport.sony.com/EN/news/article58.) To qualify for this benefit you must (1) have a Sony television model KDS-70Q006 or KDX-46Q005 and (2) have purchased a 1080p DEVICE (see definition of this term below) on or before July 25, 2008. To qualify for this benefit you must (1) have a Sony television model KDS-70Q006 or KDX-46Q005 and (2) have purchased a Sony Bluray Disc Player (excluding gaming device) between July 25, 2008 and 30 days after the Court grants final approval. To know whether you bought a 1080p DEVICE, look at this definition and the last page of this Claim Form: A 1080p DEVICE is a HD-DVD player, a Blu-Ray player, gaming device (e.g., Sony PlayStation3), or computer that can transmit a digital signal known as a 1080p signal via an HDMI interface. On the last page of this Claim Form, there is a list of devices that can transmit a 1080p digital signal via an HDMI interface. If you have one of the devices included on this list, you have a 1080p DEVICE. If you are not sure whether your video device or your computer transmits a 1080p signal, check the owners manual for the device or go to the manufacturers website and check the Specifications Sheet for the device.
SEND YOUR PROOF OF PURCHASE To receive the E-Credit that you selected above, you must attach to this Claim Form a proof of purchase as follows: If you do not provide the required proof, your Claim Form may be rejected. If you chose the $28 E-Credit or the $75 E-Credit, in Selection 2 or 4: Attach a legible copy of the original receipt for your Sony Blu-ray Disc Player.

If you chose the $60 E-Credit or the $180 E-Credit, in Selection 1 or 3: STEP ONE: Attach proof of purchase of your 1080p DEVICE using any one of the following: (a) legible and dated copy of the original receipt identifying the 1080p DEVICE; or
(b) legible copy of a dated invoice marked paid that identifies the retail seller, the sellers address, and identifies the purchase of the 1080p DEVICE; or (c) legible copy of a dated cancelled check contemporaneously identifying the purchase of the 1080p DEVICE; or (d) legible copy of a dated credit card bill that identifies the purchase of the 1080p DEVICE; or (e) legible copy of a dated shipping invoice identifying the 1080p DEVICE; or (f) a portion of the box originally holding the 1080p DEVICE that identifies the make and model number, provide the purchase date and the name and location of the retailer where the 1080p DEVICE was purchased in the spaces below, and attest to the fact that you were unable to locate the receipt for the 1080p DEVICE. Sony shall determine whether you are eligible to receive a benefit and notify you accordingly. If Sony Electronics rejects your claim because you didnt provide the required information, you have 20 days from the date on the rejection notice to provide the missing information to the address below. If you do not receive acknowledgement of an acceptance or notice of rejection within 6 weeks of when the settlement becomes final, you may call the toll-free (866) number below to confirm whether your claim was received. The E-Credits offered in this settlement may only be used once and will expire 365 days from the date this settlement becomes final. However, the E-Credits are freely transferable. Sony will mail E-Credits after the court grants final approval of this settlement, and within 6 weeks following (a) 30 days after the Court grants final approval of the settlement or (b) Sonys receipt of a valid and properly completed Claim Form and other required documentation, whichever is later. If you have more than one of the Sony televisions listed on the first page of this Claim Form, you may be eligible to receive an E-Credit for each television. To make a claim for an additional E-Credit, you MUST submit a complete claim form for EACH television. You must be a resident of the United States to qualify for any benefit.
