Important instructions, terms and conditions
To receive your TASCAM Rebate, you must follow these instructions: 1. Complete all information requested on this form. 2. Circle the qualifying TASCAM product purchases and the purchase date on your invoice. Your AMS receipt/invoice or a copy of it must be dated between March 15, 2009 and April 15, 2009. 3. Remove the original UPC code from the qualifying TASCAM product package(s). Photocopies of the UPC will not be accepted. Only new products purchases apply for rebate not refurbished, floor model without box, etc. 4. Mail this completed offer form, a copy of the AMS receipt/invoice and the original UPC(s) in a single envelope to the address below. Submissions must be postmarked by May 15, 2009 to qualify.
