WBAMC nurses share their week with staff
From May 6 through May 12 the William Beaumont Army Medical Center nursing staff celebrated Nurses Week. The first National Nurses Week was observed Oct. 11 through 16, 1954, in honor of the 100th anniversary of Florence Nightingales mission to the Crimea. It wasnt until 1974 that President Nixon declared the time between May 6 and May 12 as Nurses Week, said Capt. Kathleen Young, a registered nurse at WBAMC. Since this declaration, the celebration begins May 6 and ends May 13 Florence Nightingales birthday. She is acknowledged as the founder of modern-day nursing. Nightingales other interests included health science and health statistics, philosophy, political advocacy, evidencedbased practice, holistic nursing and public health. All of these areas are included in modernday nursing. Celebrating the theme Nursing Care Today for a Healthier Tomorrow is in keeping with the more than 350 nurse professionals at WBAMC who have the compassion to provide hands-on patient care at the bedside or during contingency operations seven days a week, 365 days a year. The celebration recognizes the past, acknowledges the present and sets the direction of the future of nursing. A nursing career is extremely rewarding and chalthe nursing staff to meet these licensure requirements. Most states require nurses to maintain their licenses through education. Nurses from all military branches can submit research or performance improvement presentations to the Phyllis J. Verhonick Research Course for consideration. WBAMC nurses submitted 16 abstracts and 12 were accepted for presentation. This effort continues to demonstrate nursing as a profession, as our practice is based upon research evidence, Young said. A leisurely dog walk in McKelligon Canyon was selected as one of the Nurses Week activities because it encourages a positive, realistic image of nurses in promoting healthy behaviors in the community. The dog walk was also an opportunity to raise funds for Pets Alive a nonprofit organization that helps fund low cost spaying and neutering, said Sarafini. This was our gift back to the community. WBAMC nurses are among the largest group of health care workers in America and contribute to the health and wellbeing of the Army and the nation by providing a healthier tomorrow through nursing today. Nursing is touching lives in a holistic manner despite difficult circumstances and austere environments, said Blunt.

Opportunities in nursing are many and include floor nursing, public health nursing, case management, research, trauma, nurse anesthetist, nursing education and more. Serafini spoke of the many opportunities that she has been afforded as an Army Nurse Corps officer, especially the coaching provided by her mentors and coworkers. She said their commitment to nursing Soldiers and families is what keeps her
going day after day. I appreciate the opportunities that the Army Nurse Corps offers to me, she said. This celebration is to raise awareness of the value of nurses in the treatment and care of patients and to educate WBAMC beneficiaries about the role nurses play in the military health care system. The nursing profession is inherently connected to our basic existence, which is to serve others and provide care to all of our beneficiaries and
their families, said Col. Ann M. Blunt, chief of the Department of Nursing. I have been a part of many patients lives and their families, and feel blessed to care for them, said Sarafini, who works in the Department of Emergency Medicine. Our goal was a celebration and education/research foci. Throughout the week there were presentations aimed at increasing the knowledge base of the staff, Young said. These presentations enabled

USASMA Strategic Communications
Initially developed to avert cardiovascular disease among senior Army officers, the Army Physical Fitness Research Institutes Executive Health and Fitness Assessment Program now offers the same benefits to noncommissioned officers here. The APFRI program, sponsored by the Army War College, includes fitness, health and mental well-being components and comes on the heels of nearly three decades of assessments offered to AWC students. It was the vision of Army Chief of Staff Gen. George Casey, Jr. who, having recognized the importance of the program, wanted to see the APFRI mission expand and partnered with Professional Military Education on the enlisted side as well. The fitness portion focuses on body composition, strength and flexibility. The health component looks at stress, cholesterol, blood pressure, nutrition and aerobic capacity. A Soldiers mental well-being is addressed via surveys and personal dialogue. Again, the focus is on health and fitness that truly benefits Soldiers. So what was a pilot program at the U.S. Army Sergeants Major Academy in 2004 now has proven valuable enough to approve 10 more mini-APFRI sites at Army senior leader courses around the nation in 2014. In 2004 APFRI studied the population attending USASMA and Command and General Staff College, said Lt. Col. Tracy Smith, deputy director of the APFRI USASMA annex. What we found was

that, despite being younger in age, the sergeant majors and majors had more issues with cholesterol, hypertension, stress and sleep difficulties. In 2008, annexes were opened in both locations. APFRI is also setting out to further enhance the fitness component of the program. They now offer a Soldier-Athlete Deployment Fitness series that takes into consideration the rigors of combat-related duty. That means working on an individuals core strength, and adding balance and agility exercises to workout regimens. These exercises provide training for Soldier fitness that is actually related to tasks in combat, said Smith. The program provides our senior leaders with the capability to maintain optimum health and fitness in a sustained, operational environment. Across all annexes, APFRI has aligned itself with Professional Military Education organizations such as USASMA, which prides itself as a leader in advancing enlisted education. We did this for two critical reasons, said Smith. First, it provides an avenue for our senior leaders to learn more about their own health and fitness, as well as the needs of their Soldiers and families. Second, its an opportunity for them to take advantage of a down turn in operational tempo for them while at school. Theyre better able to focus on their own health and fitness. Operational tempo in the Army has been a big influence on such programs. Reflecting on the past eight years of war and our deployment experience in

Afghanistan and Iraq, we can now begin to understand the individual health and resilience problems associated with our deployment tempo, said Sgt. Maj. of the Army Kenneth Preston, in a letter to the troops. And so APFRIs Executive Health and Fitness Assessment Program aims to provide the type of insight in health, fitness and nutrition that readies troops for those hardships associated with combat. As a collateral benefit, it indirectly supports Prestons focus on the Comprehensive Soldier Fitness program, which looks at five dimensions of strength validated by
the World Health Organization to include physical, emotional, social, spiritual and family. Were getting outstanding results, said Smith, regarding the APFRI program. The 80 to 85 percent of USASMA students who take advantage of the program here say its outstanding, that it provides a great deal of health and fitness insight to senior leaders. The results, its hoped, will not only go a long way toward curbing cardiovascular disease but also help prepare for combatrelated challenges from a health and physical fitness perspective.

Interval training improves cardio systems, Soldiering

Interval training is an excellent way to train the cardiovascular energy systems of the body that are required for performance of military duties, while minimizing mileage wear and tear on the lower extremities. Too much distance running has been shown to increase overuse injuries without significantly increasing fitness levels. Distance running is also less fundamental to physical tasks Soldiers actually do. Interval training guidelines for running are described here, but the same type of ratios can be applied to activities such as cycling and swimming. Interval running is performed with multiple bouts of all-out (high intensity) running interspersed with periods of recovery. High-intensity activities include sprints, shuttle runs,
and hill/stair running. Intervals are performed by adhering to a work-to-recovery ratio of 1:3 or 1:2. For example, a work-to-recovery ratio of 1:3 would be a sprint of 10 seconds followed by a relative relief period (walk or slow jog) of 30 seconds. Intervals can be intensified as fitness improves (for example, 15:45 and 20:60). A 1:2 work to recovery ratio would be a sprint for 10 seconds followed by a relative relief period of 20 seconds (progressively 15:30, 20:40 and 30:60). Interval sets could start around five and progress to a maximum of 10 by adding no more than one set every two weeks. Shuttle runs (running back and forth between two fixed distance lines) or repeated runs up and down a hill or stairs are other forms of interval running that can be conducted in similar ratios to those described above.
Interval running can be conducted individually or in ability groups. One of the best aspects of interval training is that a great workout can be performed in a little time. A good idea is to replace long-distance endurance running with shorter-interval running two of three running
days. Distance running greater than 30 minutes has been shown to increase the risk for injury, so replacing most of a programs distance running with intervals should reduce the number of overuse injuries. Besides preventing injuries, interval training adds muchneeded variety and intensity as
well as more functional speed and agility training to a program. Adding more speed and agility training to a physical training program is vital because Soldiers face the occupational need to run quickly over short distances much more often than the need to jog slowly over long distance. Below is the Tabata protocol (an aerobic cardio workout method developed by Izumi Tabata) for high-intensity interval training: Warm up - jog at 50 percent max for 5 minutes 6 x (sprint for 20 seconds plus 10 seconds rest) 1 minute rest 6 x (sprint for 20 seconds + 10 seconds rest) 1 minute rest 6 x (sprint for 20 seconds + 10 seconds rest) 1 minute rest Cool downjog at 50 percent max for 5 minutes

WBAMC prepares SFCC for transition

In a few months, WBAMC is set to open one of the largest and most modern health clinics in the Department of Defense. The Soldier Family Care Clinic will be a 146,000-square-foot health clinic at East Fort Bliss at the corner of Sergeant Major Boulevard and SSG Simms Street. Groundbreaking for the $42.5 million health clinic was in January 2009 and construction has proceeded on schedule, with completion expected in August. The SFCC was planned by evaluating the population to be served and programming future growth for Soldiers and families who will use the facility. The principal use of the SFCC will be to provide primary care to approximately 38,000 Soldiers and families assigned to 1st Armored Division and the Grow the Army Brigades at Fort Bliss. The SFCC is a patient-centered facility designed to include a variety of services frequently used by Soldiers and family members. Workflow and evolving patient need were central to the design of the SFCC and can be understood best by evaluating the location of services within the clinic. Adult and pediatric primary care located on the first floor and are expected to be the lead services offered in the SFCC. In addition to primary care, the first floor has a five-window pharmacy, a four-station lab, a digital radiographic and ultrasound suite along with a patient administration department. Services located on the second floor will include mental health, physical therapy, occupational therapy, ophthalmology, physical exams, preventive medicine, tele-dermatology, aviation medicine and TRICARE health benefits advisers. Each of these sections will be designed to offer a greater opportunity for one-stop shopping. Providing medical resources closer to where Soldiers and their families live and work will enhance access to health care while providing an environment that promotes Soldier and family resilience. Although construction will be completed in August, the first patients will not be treated in the SFCC until December 2010. Between August and December 2010, the clinic will be transitioned. During this period, the building may look as if it is ready to receive patients, but transitioning the building from active construction site to clinical treatment facility is no small feat. Transitioning includes installing furniture, computer network and medical equipment, hiring and training staff, designing workflow processes, and achieving all applicable certifications prior to treating patients. Some transitioning can begin as construction nears completion, but a large portion can only be completed once construction is complete. Many of the transitioning tasks must occur in succession. For example, the radiological furniture must be installed before the radiological computer network can be installed and tested. The radiological computer network must be tested before the radiographic imaging equipment can be installed. This scenario will repeat itself in the pharmacy, the lab and many other portions of the building. As the SFCC prepares to see patients, WBAMC will inform the Fort Bliss community of its progress and provide greater detail to the services to be offered. Ensuring our beneficiaries understand our efforts and are offered the opportunity to comment on them will help WBAMC leadership meet the needs of our beneficiaries. A concerted effort to provide information to the transition plan, enrollment guidelines and daily operations for the SFCC will continue to be published throughout the summer.
