Completely fill this form and submit via email (service@feiyueusa.com) or FAX (909-613-5808). Please note that manufacturers warranty services include labor and parts only. Shipping charges are not included. All additional costs incurred are solely of the responsibility of the party requesting the service 4. RMA form will be sent back to you with an assigned RMA number. Please include this form with the assigned RMA number with your service item. A nominal fee of $35 for return shipping charges must be included with your service item.
