To qualify for rebate, your LG refrigerator must be purchased between June 2nd and July 14th, 2006. All claims will be void if received after August 14th, 2006. If you have not taken delivery of your refrigerator and do not expect to receive it prior to August 14th, 2006, please forward your rebate form immediately, without the serial numbers. When you take delivery of your refrigerator, call 1-888-332-9450 no later than October 14th, 2006 and report the serial number. Your rebate cheque will be mailed to you after receipt of a valid serial number. All claims must include a copy of the refrigerator purchase sales receipt, a completed copy of this rebate claim form, and the serial number of the purchased refrigerator. All invoices submitted must be clear and legible copies. No responsibility will be taken for lost, misdirected, illedgible or delayed claims.
