How to Receive Your 2-Year Limited Warranty: To receive your 2-year limited warranty, purchase a new Panasonic PT-AE4000U projector from an Authorized Panasonic Dealer in the United States (or PT-AE4000U-EC from Panasonic Direct), between April 1, 2011 June 30, 2011. This cannot be combined with any other offer. Return this completed form, including the serial number label, along with a copy of your invoice with the purchase price circled. IMPORTANT: You must cut out and include the original label from the carton, which includes the UPC code, model number and serial number, or your claim cannot be processed. Offer valid for purchase made from April 1, 2011 June 30, 2011. All claims must be postmarked within 30 days of purchase. This offer is only available to the end user and original purchaser. No claims from re-sellers, dealers/distributors, their employees or warehouse facilities, groups, clubs or organizations will be accepted or acknowledged. Product must be new and un-used.
