Congratulations on the purchase of your new DELL Computer. Please complete this rebate redemption form and mail it to the address provided below and your rebate will be mailed to you within 8-10 weeks of receipt (subject to the terms and conditions in this form).
4. Mail the completed form, along with a copy of your packing slip, to the address below. YOUR REQUEST MUST BE POSTMARKED WITHIN 30 DAYS OF THE RECEIPT OF YOUR COMPUTER. Allow 8-10 weeks for processing. If you have not received your rebate cheque after 10 weeks you may call 1-866-566-0590 or visit www.rebatestatus.com. If you need another copy of this form, you can go to www.dell.ca/rebates, or www.dell.ca/fr/rebates for French version, and print a copy.

You must provide your Dell Customer Account Number, your new systems Service Tag Number (which is a 5-7 digit code located on a sticker placed on the bottom or on the back of your new computer) your order date and your order number. If you purchase more than one system please include Service Tag Number from each system.

IN SIGNING AND SUBMITTING THIS REBATE FORM, YOU ACKNOWLEDGE AND AGREE TO THE FOLLOWING TERMS AND CONDITIONS: Dell Mail-In rebate offer valid only with the purchase of the following Dell Computers: Dimension 2200, 4500, 4500S and 8200 series desktops and Inspiron 2600, 4100, 2650 and 8200 series notebooks. Qualifying purchases must be made from June 1st, 2002 to June 7th, 2002 10pm EST for phone orders and 5pm EST for online orders. Canadian residents only. While supplies last and no substitutes or rain checks. Cannot be combined with any volume or other discounts including corporate employee purchase plans or other promotional offer (except Internet Access offers, Dell Deals Online and Instant Savings). Request form must be fully completed and MUST include: a copy of the dated packing slip for purchase made direct from Dell, customer number, service tag number, and order number for qualifying product purchased. Keep copies of materials sent. Materials received become the property of Dell and will not be returned. One rebate per qualifying system, maximum of 5 systems per customer. Dell is not responsible for lost, late or misdirected mail. Request must be postmarked 30 days from receipt of system. Any claims postmarked after 30 days from receipt of system will not be processed. If terms and conditions are not met, rebate will not be sent. Please allow 8-10 weeks for receipt of cheque. Rebate cheques are void if not cashed within 90 days of issuance and cannot be reissued. The description of the qualifying Dell system must appear on a copy of the dated Dell packing slip. Only actual Purchaser of the qualifying products may participate in this rebate program. Void where prohibited or restricted by law.
