To receive your $20 rebate check, you must mail the following three items: Completed rebate claim coupon including scanner serial number and your signature.* Original UPC label with numbers cut from the ScanMaker i320 original product carton. No reproductions will be accepted. A photocopy of your store-identified sales receipt or packing slip with the purchase price circled and date of purchase clearly visible. (Please keep the original sales receipt or packing slip receipt for your records.) Envelopes containing completed coupon, UPC bar code cut from the original product carton and photocopy of sales or packing slip receipt must be postmarked no later than 30 days from purchase date. TERMS AND CONDITIONS:

1. Microtek is not responsible for lost, stolen, or misdirected mail 2 Limit one rebate per name, address and household. 3. To receive your $20 rebate check, you must mail the following three items: a) completed rebate claim coupon including scanner serial number and your signature*; b) original UPC label with numbers cut from the Microtek ScanMaker i320 original product carton. No reproductions will be accepted; c) a photocopy of your storeidentified sales receipt or packing slip with the purchase price circled and clearly visible. (Please keep the original sales or packing slip receipt for your records.) 4. Incomplete coupons will not be accepted. Coupons must have customer's signature. 5. Clearly hand print information on rebate coupon. 6. Photocopy of your original sales receipt or packing slip must be dated no earlier than December 1, 2005 and no later than March 31, 2006. 7. Envelope containing rebate submission must be postmarked no later than 30 days from purchase date. Faxed copies or copies sent via email or as email attachments will not be accepted.
8. Fraudulent submission of multiple requests or falsification of information will void this offer and could result in federal prosecution under the U.S. Mail Fraud Statutes (18 USC Section 1341 & 1342). 9. Offer valid to end-user customers only. This offer is not valid to resellers, retailers or distributors of Microtek products and their families. 10.
